• Slips, trips, and falls

• Collisions with equpment, walls; fipors, or other participants

• Muscle strains, sprains, or other physical injuries

• Serious injury, permanent disability, or death

I voluntarily assume all risks associated with FSA, it's owners, operators, staff, coaches, and affiliates from any and all claims, liabilities, including those arising from negligence, to the fullest extent permitted by law.

I understand that participation may require physical exertion and I certitiy that I am physically fit and able to participate.
